Clinical Simulation: Evaluating Insomnia in Difficult to Treat Depression
In patients with major depressive disorder, approximately two-thirds present with comorbid insomnia; in fact, epidemiological and clinical studies show depression to be the most frequent cause of chronic insomnia. However, evaluation of insomnia in depressed patients is complex; insomnia can be a core symptom of major depression as well as a side effect of antidepressants. Clinicians must also rule out other primary sleep disorders. In this clinical simulation, providers will conduct a follow-up interview with a patient taking an SSRI and atypical antipsychotic for a major depressive episode. Some symptoms of her depressive episode have improved but she continues to have significant complaints related to neuro-vegetative symptoms, and in particular sleep.
